What Is CommuniCare Advantage?

CommuniCare Advantage is a Medicare Advantage contractor that offers two Special Needs Plans (SNPs):

  • CommuniCare Advantage Chronic Condition Special Needs Plan – for those with diabetes mellitus, cardiovascular disorders, or congestive heart failure. See below for more detail.
  • CommuniCare Advantage Institutional Special Needs Plan – for those who are in institutions such as nursing facilities, or are eligible for institutional care. See below for more detail.

Our valued CommuniCare Advantage members receive personalized care from expert providers including physicians, nurse practitioners, physician assistants, and nurses. Our model of care is coordinated, proactive, and customized to members’ unique healthcare goals and preferences. Your care is our priority.

What Are the Benefits of CommuniCare Advantage?

Our service takes the hassle and frustration out of trying to coordinate care from many different providers. Members in both plans benefit from membership:

  • We ensure that members receive integrated, collaborative care from a local clinical team.
  • We guide families through navigating the sometimes overwhelming healthcare system to get their loved one the best care.
  • The insurer is part of the healthcare team, which minimizes benefit conflicts.
  • Skilled nursing services are available without the three-day hospital stay requirement
  • Members receive benefits such as vision, hearing, and dental services as part of their plan.
What Are the Benefits of CommuniCare Advantage?

CommuniCare Advantage Chronic Condition Special Needs Plan (C-SNP)

A Chronic Conditions Special Needs Plan (C-SNP) is a Medicare Advantage plan specifically created to provide targeted care to individuals with chronic conditions. CommuniCare Advantage’s C-SNP is designed around the health care needs of people with diabetes mellitus, congestive heart failure, and cardiovascular disorders.

Partnership is central to the C-SNP. Those with diabetes, heart failure, and cardiovascular disorders know that managing these chronic conditions takes work. C-SNPs are for those willing to be active participants in their care, working with their doctors and care teams to manage their conditions. This includes taking medications as prescribed, getting regular checkups, and communicating with their care team. And that care team is committed to working together and working with you on a plan that supports your health goals.

The CommuniCare Advantage C-SNP is available to Medicare Advantage-eligible individuals in our C-SNP service areas who have serious or disabling diabetes mellitus, congestive heart failure, or cardiovascular disorders.

Who Is Eligible?

The CommuniCare Advantage C-SNP service area includes three counties across three states:

  • Maryland: Baltimore City County
  • Ohio: Cuyahoga County, Hamilton County
  • Indiana: Marion County

You may be eligible for this C-SNP if you live in these service areas, are eligible for Medicare Advantage and Medicare Part D, and have diabetes mellitus, congestive heart failure, and/or cardiovascular disorders.

CommuniCare Advantage Institutional Special Needs Plan (I-SNP)

Institutional Special Needs Plans (I-SNPs) provide specialized care for those who reside in long-term care facilities. Specifically, they serve Medicare Advantage–eligible individuals who, for 90 days or longer, have had or are expected to need the level of services provided in a long-term care facility. CommuniCare Advantage’s I-SNP is available to those who meet these criteria and are living in a contracted nursing facility operated by the CommuniCare Family of Companies.

Who Is Eligible?

Residents eligible for this plan should be living in one of our nursing facilities for a minimum of 90 days or expect to be there for at least 90 days.

  • Answering these questions might help you be more certain if you qualify. A yes answer suggests that you may be eligible for CommuniCare Advantage:
  • Do you currently have, or are you eligible for, Medicare Part A (hospital insurance) and Part B (medical insurance)?
  • Have you been a resident of one of our contracted facilities in the areas listed below for longer than 90 days?

Getting Started and Maintaining a Continuum of Care

As a new CommuniCare Advantage member, you’ll be asked to tell us what your healthcare goals and preferences are and to undergo a health assessment so we know how to create a customized care plan with you. After that, our care management team will check in with you regularly to coordinate care and referrals for any specialty care needed. Our mission is to keep you healthy and prevent unnecessary and unwanted trips to the emergency room or hospital. As your healthcare partner, we’ll work closely with you and your care team to ensure coordination and care that works for you.
