CommuniCare Advantage: Your Care Advantage

CommuniCare Advantage is a Medicare Advantage contractor that offers three Special Needs Plans (SNPs):

  • Communicare Advantage Medicare Prescription Drug Plan – a provider-owned plan tailored to meet the needs of our members, aimed to offer customized care, with two plan options that cater to your personal needs. See below for more detail.
  • CommuniCare Advantage Chronic Condition Special Needs Plan – for those with diabetes mellitus, cardiovascular disease, or chronic heart failure. See below for more detail.
  • CommuniCare Advantage Institutional Special Needs Plan – for those who are in institutions such as nursing facilities, or are eligible for institutional care. See below for more detail.

Our valued CommuniCare Advantage members receive personalized care from expert providers including physicians, nurse practitioners, physician assistants, and nurses. Our model of care is coordinated, proactive, and customized to members’ unique healthcare goals and preferences. Your care is our priority.

Who Is Eligible?

The CommuniCare Medicare Advantage Prescription Drug Plan is open to anyone that qualifies for Medicare and in our service area to include these counties across three states:

  • Maryland: Anne Arundel, Baltimore, and Baltimore City Counties
  • Ohio: Butler, Hamilton and Montgomery Counties
  • Indiana: Hamilton, Hancock, Hendricks, Johnson, and Marion Counties

CommuniCare Advantage Chronic Condition Special Needs Plan (CSNP)

A Chronic Conditions Special Needs Plan (CSNP) is a Medicare Advantage plan specifically created to provide targeted care to individuals with chronic conditions. CommuniCare Advantage’s CSNP is designed around the health care needs of people with diabetes mellitus, chronic heart failure, and cardiovascular disease.

Partnership is central to the CSNP. Those with diabetes, heart failure, and cardiovascular disease know that managing these chronic conditions takes work. CSNPs help those who are active participants in their care, working with their doctors and care teams to manage their conditions. This includes taking medications as prescribed, getting regular checkups, and communicating with their care team. And that care team is committed to working together and working with you on a plan that supports your health goals.

The CommuniCare Advantage CSNP is available to Medicare Advantage-eligible individuals in our CSNP service areas who have diabetes mellitus, chronic heart failure, or cardiovascular disease.

Who Is Eligible?

The CommuniCare Advantage CSNP service area includes these counties across three states:

  • Maryland: Anne Arundel, Baltimore, and Baltimore City Counties
  • Ohio: Butler, Cuyahoga, Hamilton, Lorain, and Montgomery Counties
  • Indiana: Hamilton, Hancock, Hendricks, Johnson, and Marion Counties

You may be eligible for this CSNP if you live in these service areas, are eligible for Medicare Advantage and Medicare Part D, and have diabetes mellitus, chronic heart failure, and/or cardiovascular disease.

Plan Benefits

CommuniCare Advantage Institutional Special Needs Plan (ISNP)

Institutional Special Needs Plans (ISNPs) provide specialized care for those who reside in long-term care facilities. Specifically, they serve Medicare Advantage-eligible individuals who, for 90 days or longer, have had or are expected to need the level of services provided in a long-term care facility. CommuniCare Advantage’s ISNP is available to those who meet these criteria and are living in a contracted nursing facility operated by the CommuniCare Family of Companies.

CommuniCare Advantage Institutional Special Needs Plan (ISNP)

Who Is Eligible?

Residents eligible for this plan should be living in one of our participating nursing facilities for a minimum of 90 days or expect to be there for at least 90 days and in our service area to include these counties across three states:

  • Maryland: Anne Arundel, Baltimore, and Baltimore City, Carroll, Howard, Montgomery, Prince George’s Counties
  • Ohio: Butler, Clark, Cuyahoga, Geauga, Hamilton, Lorain, Lucas, Mahoning, Montgomery, Stark, Summit, Trumbull and Williams Counties
  • Indiana: Clark, Elkhart, Fayette, Floyd, Hamilton, Hancock, Harrison, Howard, Johnson, Lake and Marion Counties

  • Do you currently have, or are you eligible for, Medicare Part A (hospital insurance) and enrolled in Part B (medical insurance)?

Getting Started and Maintaining a Continuum of Care

As a new CommuniCare Advantage member, you’ll be asked to tell us what your healthcare goals and preferences are and to undergo a health assessment so we know how to create a customized care plan with you. After that, our care management team will check in with you regularly to coordinate care and referrals for any specialty care needed. Our mission is to keep you healthy and prevent unnecessary and unwanted trips to the emergency room or hospital. As your healthcare partner, we’ll work closely with you and your care team to ensure coordination and care that works for you.
